Infusion Billing to Medicare

We had billed 96365, 96375, A4222, A4221, A4216, J0878, J1335 and J7050 without any modifiers to Medicare on a particular DOS. Everything got paid except 'A-codes'.

They denied because of not separately reimbursable. This is a serious issue we are facing here.

Please can anybody help to getting these codes paid?
