Denials for 64483, 64484

I am billing for a Pain Management Provider. Came across so many denials from medicare for CPT® codes 64483, 64484. I billed 62290-3 Units(51), 72295-3 Units(26) (59), 64483 (50) (59), 64484(50) to medicare on a same day. Got payment for 62290, 72295 and denial for 64483, 64484.
Reason for denial for 64483 is "CO-B15, M80 (THIS SERVICE\PROVIDER REQUIRES THAT A QUALIFYING SERVICE/PROCEDURE BE RECEIVED AND COVERED)"
For 64484 is "CO 107 (RELATED OR QUALIFYING CLAIM/SERVICE WAS NOT IDENTIFIED ON THE CLAIM"
What should I do to get rid of this denial and get payment for 64483 and 64484????
Place of service is 22

Any Help would be greatly appreciated...............

Maybe the mod59 needs to be first on CPT® 64483. The only reason why 64484 is not paying is because 64483 denied. Looks like effective 10/1/12 mod59 is not allowed 64483 is inclusive of 62290.
