ICD-10 codes Z00.00 & Z00.01

Can you please tell me which code would be used in this scenario? Medicare patient comes in for their annual wellness visit and while there the physician also discusses their chronic medical problems and refills meds. Would you use Z00.01 and the dx's for their chronic problems? or Z00.00 on the awv and the chronic problem dx's on the separate e/m?
