Billing for nurse practioner

We have a nurse practitioner that has joined our practice. She will be seeing patients for her supervising physician while he is on vacation. We have 7 other physicians in the office that will be here. We are not going to bill incident to services. We will be billing under her NPI #.

Do we bill the regular E&M codes for the office visits and is there a particular modifier that should be used?

How many charts does the physician need to review for the nurse practioner? 30% or 10%.

Can you give just some general guidelines on billing for a nurse practioner? It would be so helpful
