Can 26520 be billed in quantity 4?

I have a dilemma with a surgery bill. I was given only one cpt code for 26520 open release of metacarpophanangeal joint with extrinsic tendon contractures, joint contractures and collateral ligament rupture. Is it possible to bill the cpt code with 4 units? Or can I bill the cpt code in 4 line items with modifier 59 to 2nd, 3rd an 4th line item, since technically it is 4 different areas in the digit?
