Repair of recurrent incisional hernia with removal of old mesh

Is there a CPT® for removal of old mesh, if there is no infection? Could 10121 be used with mod. 52 in addition to 49565 & 49568? Or could mod. -22 be used instead of reporting 10121, especially if the extra time for dissecting the old mesh is documented? (In this case an extra 2 hrs)
