Do you give HPI credit for the elements documented referring to the associated signs and symptoms? See example below

Mother states: no urination for the last 12 hours and fever today 100 F.

These are associated signs and symptoms that document Duration and Severity. Is this double dipping since we already give them credit for the associated sign and symptom?

How many HPI elements do you pull from the below scenario?

SUBJECTIVE
Chief Complaint
Vomiting?

History of Present Illness
Vomiting: Mother states that since last night patient have been vomiting several times (food contain) no urination for the last 12 hours and fever today 100 F, went to pcp and was sent here for iv fluids. DEHYDRATION.
