Abnormality addressed during a preventive visit for a New patient

Hello,
If an abnormality is addressed during a preventive visit for a new patient, and the abnormality is significant enough to require additional work, how would the problem-oriented E/M be used when new patient E/M's require 3 out of 3 key components?
