Observation Status

Pt have an open fracture of his finger, went to the E/R. The pt was admitted to observation overnight for preop management. His PCP went to see him the following day. Do I have to bill the 99218-99220 or 99221-99223 for that visit? CPT® book really confuses me. Thanks!
