Multiple providers billing undeer one doctor

My question is: In a practice having 2 full time MD's, a part time MD, 2 PA's and an NP, can we bill all claims under the medical director's name (one of the 2 full time MD's)?

The 2 FT MD's are each enrolled in Medicare, as is one of the PA's.

Some commercial payors don't contract with PA's or NP's, so can I just bill as incident to for these commercial payors?

Do I need to use a Q6 modifier on these claims?

Multiple providers under one doctor

You may not bill for physician a under physician b's provider number. Each must be separately credentialed and enrolled with the payers.

For commercials, billing for NPs and PAs depends on payer policy. For Medicare, search for incident to and shared services in the Codapedia data base (in the search box above "look it up" on the left" for the rules.

But, no, in general, you cannot bill for everyone under one MD's provider number. It will certainly get the payers' attention because of the volume of services billed by one MD. How can one MD bill for XX office visits in one day?
