double dipping

If we bill for a Visual Field 92083. On the exam form they filled out the visual field part. Can we also count the visual field as reviewing/ordering a tes out of the medicine section or Independent visualization of image? or is this double dipping?

Also on the Assessment and Plan part of the exam, does the plan have to be written in by the doctor? This was an issue where I am at..

A technician may administer the test (this is called the technical component of the VF) but proper documentation of the test is only complete when it includes the professional component (physician interpretation). This interpretation is usually a short narrative describing the reliability of the results and the findings, as well as the clinician's impression and assessment. All of this is included in visual field code. If you already interpreted the test, it would not be necessary to review, but you could get a point for the ordering of the visual field. The physician would then decide what the plan will be based on test results, so, yes, it is the providers responsibility to document his/her plan. This is common sense. In my opinion you will not get a point for independent visualization because it is included in procedure code that you do this.
