neurological or neuromuscular coding question

i have billed CPT® code 95999 twice time but both time insurance has rejected it. kindly guide me about it's appropriate billing procedure and about modifier which should be used with it.

This code is an un-listed procedure that requires a special report, which requires pertinent information which should include an adequate definition or desciption of the nature, extent, and need for the procedure and the time, effort and equipement necessary to provide the servide. please see the front of the CPT® book, page xiv, which explains Unlisted procedures or services.
