What is the icd-9 code for poor po intake?

Pt with Alzheimers dementia. Pt was admitted due to to poor po intake, failure to thrive. According to daughter, pt went appx one week eating sporadically. Daughter is caregiver for patient at home. Daughter request peg placement in order to continue care for her at home. PMD is referring her to GI for PEG tube.
