Need urgent help, What is the code for Allergic reaction?

Pt is having rash and itching dx is allergic reaction, what can we code for it, can we code 995.3 or else we have to code signs and symptoms

Can you please cite any authentic reference how to code it

Allergic reaction NOS
Hypersensitivity NOS
Idiosyncrasy NOS

Excludes:
allergic reaction NOS to correct medicinal substance properly administered (995.27)
allergy to existing dental restorative materials (525.66)

specific types of allergic reaction, such as:
allergic diarrhea (558.3)
dermatitis (691.0-693.9)
hayfever (477.0-477.9).

If it is an adverse effect of a drug in therapeutic use, yes

Some patients develop an allergic reaction to a drug even though it was administered and taken correctly (adverse effect). In these cases, the manifestation (rash) is sequenced first followed by an E code from the Therapeutic Use column (Table of Drugs and Chemicals). The E codes in this column are used only when a patient has an adverse reaction to a drug taken and given properly.

Thank you very Much Nancy for you help, I really appreciate your efforts.

My Team Leader (coding supervisor) is not in agreement with me. he is asserting that for an unspecified allergic reaction coding of sings and symtpoms from HPI , ROS, and PE is enough. no need to code 995.3. Can u please give me the page no where I can find the specific guidelines for coding unspecified allergic reaction in the ICD-9 coding book, We are using Ingenix 2012 ICD-9-cm book.

995.3 Allergy, unspecified
Allergic reaction NOS

Code 995.3 is used when you have no additional information. As the excludes note states NOS (not otherwise specified).
If the circumstances state the "Adverse reaction" was due to a drug or chemical, you code the symptoms followed by the E code for adverse effect.
995.27 Certain Adverse Effects Not Elsewhere Classified, Other Drug Allergy (NEC), if the adverse effects of a drug allergy are not specified.

2012 ICD-9-CM Diagnosis Code 995.3
Allergy, unspecified, not elsewhere classified. NEC means the condition cannot be found classified in other sections of the ICD-9 Tabular list or Table of Drugs and Chemicals. Short description: Allergy, unspecified. If all that is documented is "Allergy, allergic (reaction), code 995.3 is accurate. But if the cause and reaction is documented, look in Index under "Allergy".

Good morning,

I would like to revisit this topic, please. I am coding for a facility for an ER visit (think outpatient guidelines). A pediatric patient (1 year-old) has whelps & bump on face & body as presenting signs & symptoms. "Had milk for the 1st time yesterday" per Triage Report. The final diagnostic statement on the ER report is "allergic reaction/urticaria".

There is no specified linkage between the allergic reaction and milk. The ICD-9 dx codes I'm looking at are 995.3 and 708.0. After reading Nancy's posts to the original question, I'm leaning more toward 708.0 (specific Rxn).

I'm hestitant to use 693.1 because the physician doesn't even make a linkage to "food" allergy.

Any other suggestions, or have I coded this correctly, based on the final diagnostic statement?
