Billing A4566 with 97014, 97140 and 97110

we bill A4556 eith electrical stimulation and it gets denied by medicare and some commercial carriers--Medicare denies stating that pos of 11 is the wrong place of service for this codes. Commercial carriers state that the reimbursement for this medical supply is included in t he payment of other procedures. should we be billing a differenet code for electrodes rather than A4556? Thnak you

This question is not in ref to A4556. The office just purchased an EMS also. The procedure is I was told 10 minutes/session. At times, it is done twice. When the nurse stays with the patient, I am confused as to use 97014(unattended) or 97032(attended)? What code do you use to bill Medicare? I was told G0283(unattended)?
