When to use modifier 51 and 59,please differentiate with examples

Mod 51 is for multiple procedures by the same doc, same day. Put 51 on 2nd and any after. Mod 59 is for a procedure or service is not usually paid together but appropriate under the circumstances such as a separate site, separate excision,etc. Try not to use 59 if another modifier can be used.
