67145 Retinal Tears

Is it ever appropriate to bill 67145 more than once during the 90 day post op period? What if the patient has a new tear?

Is the tear in the same eye?
If the patient is returned to OR in a global period for a complication of initial surgery modifier 78 is attached to surgical code. If a different eye and unrelated to initial surgery (global) for different eye (ex, RT), then bill the procedure with modifiers 79 and (ex, LT).

The new tear is in the same eye. 67145 is a "one or more sessions" code.
