Clarification_Guidelines

Example:
A patient seen in Emergency room as outpatient on 10/18/2016 in a hospital and the ER physician refers to hospitalist on 10/18/2016. Hospitalist sees the patient on 10/18/2016 and also the specialist sees the patient on 10/18/2016.

On the next day 10/19/2016, another hospitalist discharges the patient. None of the documentation provided information that the patient is on observation.

How can we bill this service for the physicians. Can some body please help me out.
