heart cath without L ventriculography

I am confused as to which code to use for this procedure. I know that 93454 should be used when Dr does not cross the aortic valve and go into the left vent. Dr dictates recording of "LVEDP" and a pullback maneuver across the aortic valve was performed. ?? 93458 reads with or without left ventriculography. Please advise.

93458 reads with or without left ventriculography. If you do a LHC it is code 93458 (with or without left ventriculography). A LHC means crossing the aortic valve. the catheter must cross the aortic valve into the left ventricle to be a left heart cath
