Assistant Surgeon and the OP report

Are there guidelines as far as whats required for an assistant surgeon? I've always understood that the asst needs his own op report, or he can dictate a short note saying "please see Dr. A's report"...and then in that case "Dr.A's" report should clearly show there was an assistant..."we did this" or "Dr. B did this while I did this".
I have an op report that lists the asst surgeon in the header...but in the body there is a description of the procedure, but no "I' or "we"...its all "this" was done to "that".

Assistant surgeon and OP report

I know of no guidelines that require the assistant to dictate a report. The assistant's participation is usually documented in the primary surgeon's report.
