Coding feeding and swallowing treatment

Our office is mainly Speech Therapy, but every once in awhile we have a child with swallowing or feeding issues.
If seems that with Medicaid there is always a problem getting the diagnosis correct and the claims are being submitted several times until the correct combination is found. Procedure 92526
The last code 787.20, V416 denied #364 (Not in accordance with Medical Guidelines) Any input would be helpful
