critical care and vent managment

Is it appropriate when charging for critical care (E&M) that the facility can also charge for vent management under revenue code 410 (CPT® 94002/94003)? And, the same for the ED-if charging for critical care can vent management be charged under revenue code 410 for respiratory therapy?

It is my understanding that respiratory therapist duties are included in the room rate.
