Kyphoplasty CPT® code 22524

All of sudden we started getting denials on this code. Mostly Medicare. They are not even asking for the medical records to see if this is medically necessary, they are just denying outright. Does anyone know if something has changed or have been revised that I am not aware of? Please advise..

Have you checked for a new LCD?

They denied as deemed not medically necessary. Should I be billing 72291 with it?
