Office Visit then Hospital Visit

If the patient was seen in the office and the physcian sent the patient over to the hospital on this date of service. Patient went over to hospital at the ER, then was seen by one of our physcians up on the floor. Different physcian from the same office same specialty same DOS.

What is the proper way to bill for this?

I would bill one visit under who? Can we include the documentation from the office visit and the hospital and make this one charge? Should time be documented?

No office visit code, code hospital admit taking into consideration the notes in office and on floor when admitted. Probably bill under admitting doctor.
