Modifier 27

Hi- When can you use modifier 27? Is it only appropriate to be append onto E/M codes billed on a UB (the facility charging for the multiple usage of the room/etc for the E/M visit)? or can it also be billed by physicians who rendered the services the patient to indicate that the encounter was separately identifiable?

Modifier 27 is used only by facilities on the UB when more than one E & M service is provided on the same day. May also require condition code G0, usually when services are from the same cost center.
