Personal history of polyps / screening colonoscopy

I have read and re-read all the other questions about this. I am confused and I am sure I am not the only one.
Patient comes in for a 5 year screening the V12.72 is the office visit. Billing the precedure do I use the V12.72 in addition to the V76.51 and then the findings 562.10 and 455.0 with the CPT® G0105?

The reason for the colonoscopy is for the personal history of polyps. I would bill it V12.72, 562.10 & 455.0 with G0105.

The term surveillance in ICD-9-CM indicates a condition that has been treated in the past and is no longer present but is being followed. This, in combination with the fact that a screening exam is defined as an exam in a seemingly well individual with no presenting symptoms. An appropriate V-code (for surveillance and a V code for history-of) to capture the fact that the patient has a history of colon polyps will support the coding of the high-risk screening colonoscopy code.

So you are saying to use both V codes with the findings and the G code?
