What is the CPT® and a ICD.9 code(s) for a failed version attempt?

There is no specific code for a failed version attempt. You can try to file using CPT® code 59412 with a modifier 52 or 53 using ICD 9 codes 652.0X -652.9X. However if the doctor ended up doing a ceserean delivery I do not think it will be paid. Hope this helps.

Per AAPC Coding guidance, the version does not have to be successful to report 59412 and if it is not successful, it is not appropriate to add a modifier 52, Reduced Service since the provider does in fact perform the procedure. Supporting/aligning I-9 codes: 652.10, 652.11, 652.13, 652.21 and 652.23.
