coding 11900, J3301

I am having trouble submitting these to codes to Medicare. The claims are rejected by my Clearing house. I have entered the mg of medicine on the J3301. I was not sure if I had to enter the mg of medicine on the 11900 that the doctor injected in the patient.Payor ID: NYR2 Ticket number: 219195 Claim Rejected ANSI Edit: Acknowledgement / Rejected for relational field in error Detailed description of service.
