Diagnosis code for a septoplasty

We are an ENT practice in Pittsburgh, PA. One of our major payers is denying a septoplasty charge (CPT® 30520) for a deviated nasal septum (470). Can anyone tell me if there is another diagnosis code I could use? I don't have a problem with any other payer, just this one.

Any of the above should get it covered. We are an ENT clinic in Minnesota.
