coding for PT and OT

Is it ok to code a therapy session without the chronic condition ICD-9 originally identified on the physicians script? In some cases the therapist is only using the treatment diagnosis. In all cases the chronic condition and the treatment diagnosis are in the documentation and the plan of care. Do we need to include the chronic condition diagnosis and the treatment diagnosis on every claim? Is so is the chronic condition the primary diagnosis?
