remittance adjustment code

I have been receiving rejection codes that state "at least one remark code must be provided (maybe comprised of either the NCPDP reject reason code, or remittance advice remark code that is not an alert)". I cannot find an explanation for this that I can understand. Help please - what do I do with this?

I receive this rejection code for Anthem for CPT® code 80101. I know that this is no longer a valid code for them, so I have to re bill the correct G-code. Check the CPT® code and insurance and see if the code is still valid with the insurance carrier.
