When coding an extensive surgical procedure, is is best to use pre-op or post-op diagnosis codes ?

I don't know what the extensive surgery has to do with the question.

Take a look at the instructions in using ICD-9 codes. Use a symptom or sign if there is no known diagnosis. If there is a known diagnosis, use the known diagnosis.
