Level 4 office visit

Has anyone heard that seeing a patient for a particular diagnosis for the first time automatically qualifies for a level 4.

No. The person who told you that may be thinking of the CPT® vignettes; however, the bottom line is that the choice of coding is based on the actual documentation.
