29898 & 29894 together

If the surgeon does 29898, and during the scope, removes (with difficulty) a large, over 1.5 cm fragment, is a 29894 justified? In some cases, the incision has to be extended, but it is not a completely different approach. Would you recommend using the 29894 or using a -22? Thanks for your thoughts!

these questions are difficult to address without an op note, but, a debridement works by removing areas of loose, mechanically redundant cartilage (joint lining) and inflamed tissue (synovitis) from the joint. The question is, was the fragment a result of the debridement procedure? If so code 29898 is only code appropriate. The modifier -22 is used to denote a significant increase in the physician work above what is usually required for code 29898. Documenting the usual time for this procedure and then the prolonged time involved to remove a large fragment caused by the debridement technique, would be required.
