Ligation of AV fistula

Patient had AV fistula created for renal dialysis. Problem resulted in physician ligating the AV fistula. ICD9 leads you to 39.53 BUT the excludes note indicates 39.42 is correct since this was created for renal dialysis. But the code for 39.42 indicates revision - not repair/ligation. Any thoughts on correct code?
