Application of Fixation Device Maxillary and Mandibular

When billing 21110, should a provider(dentist) be billing this code twice to indicate Maxillary and Mandibular or is this code inclusive of upper and lower jaw. Common sense tells me you can't wire a jaw together without including upper and lower and you should only bill it once however... Any help or direction to websites for answers would be appreciated.
