Billing anesthesia modifying factors

If you have a CRNA being medically directed by an Anesthesiologist, and the CRNA places a CVP line, do most of you bill both the anesthesia procedure and the CVP to both the MD and CRNA, or do you bill the surgical procedures (modifying factors) under the MD????
