Z09 Encounter for follow-up when treatment is Completed

Under code Z09 the instructional note states " use additional code to identify any applicable history of disease code (Z86- Z87)

Our patient was treated for cough & vertigo, then returned to the office & the doc notes both cough & vertigo resolved but wants pt to return in 3 mths to re-check the vertigo with use of meclizine.

When coding the F/U visit for resolved issues, can we list the cough & vertigo after Z09?

Or do we have to find a personal history code of vertigo (i.e. ear)
