Icd 10

Will the HPI need to include a more specific detail to support the new ICD10 code.

There is no requirement that the support for the code be in any specific section of the notes. This didn't change from ICD-9 to ICD-10.

The documentation should support the code, but it might be an abnormal lab (supported by lab) or an HPI element, or an abnormal exam element or a discussion/description in the assessment or plan.
