Office visit & ER visit and/or hospital admission same day

If the patient is seen by their Primary Physician in the office and the visit turns into a comprehensive exam after which the provider deems it necessary to send the patient to the Emergency Room which sometimes results in Hospital Admission, is that office visit a billable visit?

If the patient is sent to the Emergency Room but not admitted is the office visit billable?
