doctor given 59400 and 59160 CPT® codes
and dx code v22.1 and 666.10
My question is should i use modifier to 59160
can i bill like following
v22.1 59400
666.10 59160 59 modifer.
Is it correct?
Actually insurance is PMG medical group(HMO)
