Same provider 2 specialties

We have a provider who has 2 specialties. He saw a patient as a consult today in Rheumatology and he wants to refer this patient for a consult to Allergy which is his other specialty. How should the 2nd consult be reported?

If he is consulting, he is not treating only making recommendations to requesting physician. A consultant does not usually make referrals the primary does.

The second visit is an established patient visit by both CPT® and CMS rules.
