Hpi

If a doctors says they came in for diabetes, hypertension, heart disease and they are stable is this enough for a HPI? This has been a agruement in our office.

This tends to be rather insufficient. Refer to the CPT® manual under the E/M section. There is lists under the History of Present Illness heading the 8 possible criteria. If you can't pull them out of that statement I would say no.

You could use stable as a severity.
