LPN billing for 99211
Can a LPN bill for the service 99211 and does the doctor have to be on site as the service is performed?

Essentially, LPNs do qualify as "non-physicians" for nurse visits. And there MUST be "direct supervision" - defined as the physician being present on site, if not in the room where the visit takes place - for 99211 to be billable. Remember, 99211 is an E&M code: there has to be some evaluation and management going on under physician supervision.
