Creatinine Clearance

When billing for creatinine clearance( CPT® 82575 ), does that include the blood and urine creatinines or can those two cpts also be billed( 82565 and 82570)?

Blood creatinine should be ordered and sent to the laboratory during urine collection. Both blood and urine creatinine measurements are necessary to calculate creatinine clearance. 82575, 81050
