New Testosterone CPT® code Fee ~ how to bill?

We are PCP office and give a lot of Testosterone injections. The old CPT® code J1080 has been deleted. We have found the new one is J1071 but cannot get a fee schedule showing the 1 mg dose and how to bill when we give 300 mg of testosterone. How do we bill this one?

I found them under my medicare dme carrier. It appears that they are being indentified as DME. I would go to your Medicare DME website and look it up there. I am in FL. We are in Jurisdication C and MAC is CGSMedicare.
