medroxyprogesterone 150mg

With the deletion of J1055 in 2013 I am asking which code other offices are using for this drug. It is always chanllenging when a specific code is deleted with out an appropriate replacement.
I recently had a denial using J3490 (NDC code was submitted with this unlisted code). I called a local GYN office and they are J1050 but are getting denials also.
The other part of this issue is -- should the NDC UOM be units or mg. and should the NDC units be 1 or 150mg. Thanks
