Documentation requirements to code 45520?

Doctor removed fecal impaction in office w/o anesthesia, it is documented that this is related to rectal prolapse, what are the documentation requirements to bill the charge 45520 does anyone know? I have read a bit on this and some people say its part of the E/M and others say the 45520 can be used but I am not finding exactly what needs to be documented in the procedure note.
