First time here,I need help,i coded 33263- Removal of pacing cardioverter-defibrillator pulse generator with replacement of pacing cardioverter-defibrillator pulse generator; dual lead system and add on code 33225 Insertion of pacing electrode, cardiac venous system, for left ventricular pacing, at time of insertion of pacing cardioverter-defibrillator or pacemaker pulse generator. I am getting an error-Claim lacks require device code for Medicare. What is the device code i'm missing?

Code 33263 only requires device HCPCS code C1721, in an outhospital setting.
Code 33225 is reported when the lead placed within the coronary sinus is connected to a new pacemaker generator or ICD. This code is also reported when the existing pacemaker generator or ICD is removed and replaced or upgraded.
According to the AMA 2012 CPT® book, 33225 can not be billed with 33262-33264 which is for the ICD generator removal and replacement. But, this was an AMA error, which they corrected in their CPT® Code Book Errata. Here are the changes:

(Use 33225 in conjunction with 33206, 33207, 33208, 33212, 33213, 33214,
33216, 33217, 33221, 33222, 33228, 33229, 33230, 33231, 33233, 33234,
33235, 33240, 33249, 33263, 33264)
