Specify substance or drug

Hello everyone!

The CPT® book states "(specify substance or drug)" -- How do I specify the substance/drug if I'm billing a HCFA-1500?

If someone has a guide to billing for threrapy IVs (not in a facility) I would be sincerely grateful.

Box 19 - This is used when an insurance carrier/code requests specific information that is not specified on the HCFA claim form.
